Electronic submission of awards applications is encouraged. A completed application consists of:
- A completed nomination form. For electronic submissions, the nomination form is available to complete online.
- A letter of nomination relating the specific criteria for the award. Nominations that do not meet the award criteria will not be accepted. This letter should identify the nominee and the award.
- One professional photograph of the nominee (head and shoulder view) with the name written on the back. Color photographs are preferred.
For electronic submissions, complete the nomination form online, attach the nomination letter as a word processing document, and submit electronically. Mail or email the photograph to the ANNA National Office. Emailed photographs must be 1 megapixel (approximately 800 x 1024 pixels) or higher resolution.
For mail submissions, download [PDF], complete, print and submit 1 copy of the completed nomination cover sheet, 1 copy of the original letter of nomination, 1 blinded copy of the letter of nomination, and the photograph to the ANNA National Office. (To be blinded for review, all identifying information about the nominee and yourself must be deleted. The name, institution, place of employment, chapter, city, and state must be removed.)
Suggestion: Compose the letter and save it as a document on your computer. Then, using the “original” document, replace all identifying information with <empty brackets> and save it as a different document.)
