Leah Foster Smith MSN, APRN, FNP-BC, CNN-NP, FNKF, FANNA, FAANP

Director of Advanced Practice Providers, Metrolina Nephrology Associates

Job Position and Professional Responsibilities:

I am a board-certified family and nephrology nurse practitioner with over 24 years of experience. As Director of Advanced Practice Providers at Metrolina Nephrology Associates, I lead the largest nephrology APP team in the U.S. and helped develop one of the first APP-led CKD clinics, now serving over 30,000 patients. I am Co-Founder and Chief Education Officer of Interactive Nephrology. I also serve as an APP Practice Consultant for Clear Edge Medical Management and as an Adjunct Lecturer at UNC-Chapel Hill’s School of Nursing. I am actively shaping the future of nephrology care delivery, leadership, and APP integration nationwide.

Describe your qualifications for this role, and include past ANNA leadership positions:

Over the past 24 years, I have built a career grounded in patient-centered care, clinical innovation, and advanced practice leadership. As an educator and nationally recognized leader in nephrology APP development, I currently serve as Director of Advanced Practice Providers at Metrolina Nephrology Associates, where I developed and lead the nation’s largest nephrology APP team. I helped launch one of the first APP-led CKD clinic models, now serving over 30,000 patients, which has been replicated in nephrology practices across the country.

A key part of my professional mission is identifying national gaps and building solutions. I co-founded Interactive Nephrology to meet a pressing need for structured nephrology APP education. There, I created Ready for Rounds and Cleared for Clinic, the first two nationally accredited nephrology APP training programs. These initiatives not only prepare new providers to deliver high-quality kidney care, but also help address the nephrologist shortage by expanding the trained APP workforce.

I am a sought-after national educator, having delivered over 80 invited nephrology lectures and workshops across the country. I hold Fellow status with ANNA (FANNA), AANP (FAANP), and NKF (FNKF), and was selected by invitation as an AANP Approved Provider.

Currently serving in national leadership roles with ANNA, ASN, NKF, and RPA, I bring a unique ability to align national nephrology initiatives with ANNA’s mission and goals. I am well-positioned to support ANNA’s strategic vision, expand member engagement, and champion innovation and growth.

Describe your role in three ANNA projects/activities which support(ed) ANNA’s mission, goals, and objectives:
Education and Professional Development

Since 2015, I have served on ANNA’s Conferences Committee, first as a member, then as Chair Designate, Chair, and now Past Chair. I’ve led national planning for ANNA’s flagship events, ensuring high-quality, diverse education across roles, specialties, and career stages. Prior to my national involvement, I contributed to the ANNA North Carolina Statewide Symposium Planning Committee (2011-2018), expanding regional education for nurses and APRNs. I have served as Content Expert Faculty and Presenter at more than 40 ANNA podium events, part of a broader record of over 80 invited nephrology presentations nationwide.

Leadership and Chapter Engagement

From 2010-2022, I served in continuous leadership with the Greater Charlotte Chapter, including roles as President, Secretary, and Health Policy Representative. I continued that trajectory of service nationally as Conferences Committee Chair Designate, Chair, and now Past Chair (2021-2026), reflecting a sustained commitment to ANNA’s mission at the local, state, and national levels.

Workforce Development and Practice Advancement

As APRN Section Editor for the ANNA Core Curriculum for Nephrology Nursing (7th and upcoming 8th editions) and author of the APRN Hemodialysis chapter, I’ve contributed to clinical relevance and academic rigor in key educational resources. I also authored the Advanced Practice chapter in the forthcoming Career Pathways in Nephrology Nursing, a resource to strengthen recruitment, retention, and the nephrology pipeline. These projects help equip nephrology nurses through education, leadership, and innovation supporting ANNA’s growth and mission.
