CLINICAL SERVICES COORDINATOR/NURSE PRACTITIONER – DIALYSIS
Are you a compassionate nurse who considers a patient's overall well-being and takes a holistic approach to their care?
Under the supervision of the Medical Director, you will ensure that the dialysis patient population receives high quality primary care and complete medical services. You will assess and treat patients and refer them to specialty care or diagnostic services.
Here is what you will do:
- Provide primary care to dialysis patients by conducting health assessments and examinations and managing common acute and chronic stable conditions.
- Identify problems and formulate treatment plans to include health education and counseling.
- Order, administer and analyze diagnostic tests and provide preventive vaccinations if needed.
- Collaborate with healthcare professionals to ensure continuity of care.
- Participate in QI to improve clinic efficiencies and patient quality of care.
- Maintain medical records, keep credentials current, and ensure a safe environment for patients and staff.
What qualifications you will need:
- Bachelor’s Degree in Nursing from an accredited college or university.
- Master’s Degree in Nursing – Adult Nurse Practitioner.
- Minimum of 5 years Post Master’s degree experience as an Adult Nurse Practitioner, including experience providing primary care to children of a variety of ages.
- Current New York State license as an RN and NP-Adult Health; and BCLS certification.
- Proficiency with computer-based programs including Allscripts, EPIC and MS Office.
- Nursing leadership ability and excellent written and oral communication skills.
